Please indicate if your dancer would be interested in performing in additional group dances (this will result in additional fees and rehearsals):
Please indicate if your dancer would be interested in performing in a solo or duet/trio (this will result in additional fees and rehearsals). Dancers must have been in Comp Crew a minimum of one year for a solo opportunity.
